Class Notice

1. I consent to join the collective action lawsuit filed against Panera, LLC to pursue claims of unpaid overtime wages during the time I performed work as a Bakery Training Specialist during the period beginning up to three years before the date this Consent to Join Lawsuit Form is filed with the Court.

2. I understand this lawsuit is brought under the Fair Labor Standards Act and I agree to be bound by the Court's decisions.

3. I designate Brown, LLC as my attorneys to prosecute my wage claims.

4. I consent to have Plaintiff and his counsel make decisions on my behalf concerning this litigation, including settlement, Attorney's fees, and all other matters.
